A not-for-profit Community Interest Company

Make a referral

Anyone can refer: professionals, family members, friends or the person themselves. Fill in the form below, or call us first to talk it through.

Before you start

It helps to have:

  • the person’s agreement to the referral
  • a short summary of their current housing
  • an idea of the support they need
  • contact details for anyone else involved, such as a care coordinator

Urgent help

We are not an emergency service. If someone is in immediate danger, call 999.

For urgent mental health help, call NHS 111 and choose the mental health option, or call Samaritans free on 116 123 at any time.

Referral form

Please give us enough to start a conversation. We will ask for support plans, risk assessments and other detailed information by secure means once we have spoken to you, so please do not include medical records here.

About you

For example: care coordinator, housing officer, family member, or "I am referring myself".

About the person needing accommodation

For example: Nottingham City

For example: managing a tenancy, budgeting, attending appointments, English language, getting into work or education.

What happens next

We aim to reply to every referral within one working day.

1

We contact you

We call or email to confirm we have the referral and ask anything we need to know.

2

A conversation

We speak with the person being referred, and with the referrer, about needs, risks and what they want.

3

Viewing

If we have a suitable room, we arrange a viewing so the person can see the home and meet the team.

4

Moving in

We agree a move-in date, sign the agreement together and start the support plan from day one.
